Operational Components of Intensive Care Coordination Using Wraparound for Youth with Complex Needs: State and Local Examples

In this webinar, representatives from the Center for Health Care Strategies (CHCS), Human Service Collaborative, and the National Wraparound Initiative will present in-depth profiles of states and localities that have taken wraparound care coordination to scale for various populations of youth with complex needs and their families. The webinar will present detailed descriptions of the organizational, operational, and fiscal structures of these approaches, including descriptions of:

Populations of Focus; Methods to conduct Eligibility and Screening, Requirements and Caseload Ratios for Care Coordinators and Supervisors; Roles of System Partners and other Professionals (e.g., Parent/Youth Peer Support Partners, Psychiatrists, Other Providers); Financing Approaches; Staff Training and Development; Provider Networks; and Evaluation, Monitoring and Outcomes

In addition to a review of state and local approaches, major themes and implications for system development elsewhere will be discussed, and a new resource summarizing operational details about approaches across the country will be introduced.
